Exosomes have quickly become one of the most talked-about innovations in aesthetic medicine. From celebrity facials to regenerative skincare serums, the word now appears everywhere, often positioned as the future of skin rejuvenation.
But behind the glossy marketing lies a more nuanced reality. At The Rejuvenation Space, we believe in understanding the science before we recommend anything to our clients. So what are exosomes, what does the evidence actually say, and how do we think about them in clinical practice?
What Are Exosomes?
Exosomes are tiny extracellular vesicles, roughly 30 to 150 nanometres in diameter, that are released by cells as part of normal cellular communication. They carry proteins, lipids, and genetic material (including mRNA and microRNA) from one cell to another, acting as biological messengers.
In the context of skin, exosomes derived from stem cells, particularly mesenchymal stem cells (MSCs), have attracted significant research interest. The theory is that these vesicles can deliver regenerative signals to skin cells, stimulating collagen production, reducing inflammation, and accelerating repair.
What Does the Research Say?
The early research is genuinely interesting. In vitro studies (cell culture) and some animal studies have shown that MSC-derived exosomes can stimulate fibroblast activity, promote wound healing, and reduce inflammatory markers. These findings have understandably generated excitement.
However, the clinical evidence in humans, particularly for cosmetic applications, is still limited. Most published studies are small, lack control groups, or use exosomes in combination with other treatments (such as microneedling), making it difficult to isolate the effect of the exosomes themselves.
The regulatory landscape is also complex. In many countries, including Australia, exosome products occupy a grey area between cosmetics and biologics. The sourcing, processing, and standardisation of exosome preparations vary significantly between products, which makes comparing results across studies and products difficult.
The Marketing Problem
The challenge with exosomes is that the marketing has significantly outpaced the evidence. Products are being sold with claims that are not yet supported by robust clinical trials. Consumers are paying premium prices for treatments whose mechanisms and outcomes in real-world skin are not yet fully understood.
This does not mean exosomes are ineffective. It means we are at an early stage of understanding, and the honest answer is that we do not yet know exactly how much benefit they deliver, in which formulations, at what concentrations, and for which skin concerns.
Our Position at The Rejuvenation Space
We follow the evidence. We are genuinely interested in exosome science and we monitor the research closely. When the clinical evidence matures to a point where we can confidently recommend exosome treatments to our clients, we will.
For now, we focus on treatments with a strong, established evidence base: skin needling, Rejuran polynucleotides, PRP, LED photobiomodulation, and medical-grade skincare. These are treatments we know work, and we know why they work.
